Become a DOMEzero provider by following these easy steps:

1. Create a DOMEzero account and register for the DOMEzero core training class. Note course fee is $750USD.
2. Take the DOMEzero core training video course.
3. Schedule a new provider interview by using the form below. Please list a few times you are available. Expect a text or email to confirm your interview time. Interviews will be with Melissa and one or more of the doctors. Please include your cell number so we can communicate by text.
